Insurance & Fees

Clear Focus Psychiatry accepts most major insurance plans in Oregon and Washington for telehealth psychiatric services. Self-pay patients are welcome with transparent pricing. Insurance coverage is verified before your first appointment so there are no surprises. A sliding scale is available for qualifying individuals.

What Insurance Plans Do You Accept?

I am in-network with the following insurance plans in Oregon and Washington:

✓ Aetna

✓ Anthem Blue Cross Blue Shield

✓ BlueCross BlueShield

✓ Cigna

✓ Premera Blue Cross

✓ Regence Blue Cross Blue Shield

✓ UnitedHealthcare

✓ UnitedHealthOne

Multiple plan types accepted including PPO, EPO, POS, Open Access, and Choice plans. Contact me with your specific plan details and I'll verify your coverage before your first appointment.

Don't see your plan? I may still be able to help with out-of-network benefits — see below.

What Are the Self-Pay Rates?

Service Duration Rate
Initial Psychiatric Evaluation 60 min $375
Medication Management — Complex 45 min $250
Medication Follow-Up — Standard 15–30 min $200

Payment is due at the time of service. Credit cards, debit cards, and HSA/FSA cards accepted.

What If I Have Out-of-Network Benefits?

If your insurance plan is not listed above, you may still have out-of-network mental health benefits. Here's how it works:

  1. You pay the self-pay rate at the time of service
  2. I provide a superbill (detailed receipt with diagnosis and CPT codes)
  3. You submit the superbill to your insurance for reimbursement

Many patients with out-of-network benefits receive 50–80% reimbursement. Call the number on the back of your insurance card and ask about your out-of-network mental health benefits.

Is a Sliding Scale Available?

Yes. A limited number of sliding scale spots are available for patients who are uninsured or experiencing financial hardship. Please mention this when you submit your inquiry and we can discuss options.

Your Right to a Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges before receiving services. This applies to self-pay patients and those not using insurance.
